Documentation

  • To enroll in an in-person college course, students are required to submit their MMR immunization records directly to ASU via their Health Portal.
  • If students are under the age of 18, prior to enrollment in an on campus course, Arizona law requires parental consent to provide medical treatment for individuals under 18, and your parent or guardian must complete a Parental Consent form.

Student Safety/ Student Experience

  • The Parent/Guardian acknowledges and accepts that participation in college courses on campus carries inherent risks, including but not limited to accidents, injuries, or illness. The Parent/Guardian agrees that the student’s participation is voluntary, and they assume all risks associated with such participation.
  • The Parent/Guardian hereby releases, discharges, and waives any and all claims, liabilities, damages, or expenses, whether known or unknown, arising out of or related to the Student’s participation in college courses on campus. This release includes, but is not limited to, claims for personal injury or property damage.
  • The Parent/Guardian understands that ASU/ASU Prep may not provide constant supervision for the student and agrees to ensure the student’s compliance with safety regulations and guidelines.
  • The student agrees to act responsibly and exercise caution while on the college campus, attending classes, and participating in college-related activities.
